Here is an older gentleman who came to our office with missing upper-right back teeth and an old porcelain-metal bridge on his front right side teeth that no longer matched his natural teeth on the left. His main concern was replacing his missing teeth and improving the appearance of his front teeth. Dr. Lawton gave the patient several options to replace the missing teeth and the patient decided to avoid any surgeries and went with a removable partial denture. His front teeth were prepared for crowns and a new bridge on the front right teeth that matched his lower teeth and his overall appearance. A high-strength porcelain material was selected to make his new front teeth. These new crowns and bridge would fit with the future partial denture. Dr. Lawton designed the partial denture to fit on the teeth without putting any wires on the front teeth to hold it in place. The patient was happy with his smile and his improved ability to chew food on the right side.

Dental Implants
This patient had many very old crowns which were now unattractive. She had severe wear on her lower front teeth plus generalized recession. She had broken off her upper right central crown and the tooth needed to be replaced with an implant. The before photos show a temporary crown on the implant. Treatment consisted of new crowns on the upper teeth and implant plus crowns on the lower front teeth. The lower back teeth will have new crowns done in the future.

This patient was born with a cleft lip and palate that was repaired as a child. The repair was incomplete which complicated his dental care and affected his appearance. Embarrassed by his smile, he did not take good care of his teeth. Full mouth extractions were done with simultaneous implant placement and delivery of immediate upper and lower dentures. Following healing, the dentures were relined and connected to the implants which help to retain the dentures securely.

This 65 year old male was not happy with his smile due to the excessive wear on his teeth. His wear was caused by a combination of acidity from gastric reflux and grinding/clenching. His lower teeth were so short that the front teeth were not even visible when he closed all the way. We did a full reconstruction to restore his smile as well as make all of his teeth taller to create a proper bite. We used a combination of veneers, partial coverage ceramic onlays, and crowns as needed that are bonded to the teeth. This allows for a durable, aesthetic result without the need to aggressively grind away good tooth structure as is done for conventional cemented crowns.

This 60 year old woman presented with considerable recession as a consequence of gum disease. Her gum disease had been successfully resolved with periodontal therapy, but her smile aesthetics left a lot to be desired. She was referred to me to restore the four front teeth following extractions and implant placement. I suggested keeping the now healthy teeth and crowning them, as extracting them and placing implants would most likely result in more gum tissue loss and not as nice a smile result. Veneers were done on the two canines.

This young man had teeth that were too small for the size of his mouth as well as some tetracycline staining which make his teeth look dark. There were also some discolored and missing fillings. Orthodontic treatment (braces) was done first, followed by gum surgery to move the gum line higher up on the teeth. Once that was healed, porcelain veneers were done on ten upper teeth. This 49 year old male has a large gap (diastema) between his front teeth. He had bonding done in the past with tooth colored composite filling material but it would chip and discolor. He wanted the space closed with something stronger and did not want to have braces. The bonding was removed and the lower right photo shows the true size of the gap as well as some smaller gaps between the central and lateral incisors. Due to the size of the gap between the centrals, composite bonding was used to close the small gaps on the laterals so as not to make the centrals any wider than needed. Ceramic bonded veneers were done on the centrals to provide a strong, aesthetic match to the rest of his teeth.

The patient is a 32-year-old female who presented with uneven and discolored front teeth. Although she had orthodontics to try to straighten the teeth, the right front central incisor was ankylosed, which means fused to the bone, so it would not move. Normally, this is a result of some trauma to the teeth during the ages when the teeth are still erupting, but the patient could not recall any. Consequently, the front teeth were different lengths, and the midline was tilted to the right. The right canine and lateral incisor were much yellower than the other teeth and would not respond to whitening. They also had some old, stained fillings, and the lateral had a root canal filling, which can discolor a tooth as well. Ceramic veneers were done on the front six teeth to correct the misalignment of the teeth and mask the discoloration of the yellow teeth.

This young patient had neglected her teeth for many years. She needed a kidney transplant but needed her infected teeth removed first. Immediate upper and lower dentures were made and the teeth removed. She has had her transplant and is doing well. All of her dental treatment was provided free as a charity case through the American Fund for Dental Health.

This patient was missing many upper teeth and had worn a removable partial denture for many years. The remaining back teeth were failing. The teeth were removed and six dental implants were placed for an immediately loaded temporary bridge. Following a few months of healing, a permanent porcelain fused to zirconia full arch bridge was placed. (Ceramics by Alf Howald, CDT)
